Community Survey Web Platform

I need a responsive HTML-PHP-MySQL site that encourages community engagement by letting residents open user accounts and complete a short survey, while admins oversee everything from the back end.

Core workflow
• Account system with secure registration, login and password reset for two distinct roles—Admin and User.
• Survey form that captures three input types: simple yes/no answers, open-ended text responses, and image uploads for supporting documents.
• All submissions stored in a well-structured MySQL database.

Reporting requirement
From the admin dashboard I must be able to choose any town in the United States and instantly see a single-page summary showing total counts and percentages for each survey question. The page should be printable and sharable via a clean URL.
