I need someone to build an application where you can enter data under certain fields and then the program compiles all of that into a formal report (either Microsoft Word or PDF (ideally Microsoft Word)) following a fixed template.

I am looking for a skilled developer to build an application that will allow users to enter text data into certain fields and then the program compiles all of that into a formal report. The application should be compatible with the Windows operating system. I have no preference for the programming language used to build the application. The main purpose of the application is to compile all the entered data into a formal report following a fixed template, preferably in Microsoft Word format. This application will be used by several people who work remotely, so users must be able to log in and enter data from their location. Application needs to be able to store and save data for future use. Honestly have no idea in regards to total cost so would like to obtain a few quotes
