Need someone to configure a customer database that has form view

We are a Driving test company that tests 3 types of vehicles (Automobile, CDL, and Motorcycle). We have been using the ancient Microsoft Works Database, but since we upgraded our computers, we can no longer use that program. We are looking for someone to set up a new database (maybe Access or Libre) that we can search for records and print off each customer's appointment details. Because we test 3 types of vehicles, we may need 3 separate databases, unless there is a way to combine them. An example of one of our forms is attached.
