Interactive Questionnaire Creation
Budget: $30 – $250 USD
I'm seeking a professional who can help design a structured questionnaire consisting of 16 multiple-choice questions, with one correct answer per question.
Key Requirements:
- Each question should be presented on a separate page, enhancing user focus and engagement.
- Users should navigate through the questionnaire using both 'Next' and 'Previous' buttons, allowing for flexible pacing.
Ideal Skills:
- Experience with questionnaire design and user interface (UI) principles
- Proficiency in relevant software/tools
- Ability to deliver a seamless and interactive user experience.
Description of the Excel Attachment:
- The text in BLACK are the questions
- The text in RED are the possible answers on that particular page
- The text in GREEN is a bar that shows the percentage of the questionnaire that is complete. It should be a green bar.
- NOTHING ELSE on the outline should appear on any of the pages. That is to help explain the outline and flow. Please DM me if this isn't clear
- I would like each page to have the same attractive single-color background. The text colors described above are NOT the colors that I want to Question & Answer text. Those texts can be the same, but should look good with the background

- I do not know the best way to obtain, store & distribute the data that is collected. Perhaps SharePoint? Some other interface? I would welcome any suggestions
