Database list management

I’m launching a mailing campaign in early February to 10,000 doctors from an online database. I am looking to hire a consultant to download as many doctor listings from the database as possible, remove duplicates, change the name from ‘last name, first name’ to ‘first name_last name’, and put the list into a format that my direct mail house can use to create custom letters/envelopes.
