I'm looking for a skilled developer to create a comprehensive patient management system using Python and MySQL. The system should include forms for creating, updating, and deleting patient details, as well as functionalities for collecting detailed patient information, generating reports, and visualizing data.

Key Requirements:
- Develop forms for managing patient details, including personal information, medical history, and insurance details.
- Personal Information fields: Name and Contact Information, Date of Birth, Address, Height, and Weight.
- Medical History fields: Past Illnesses, Current Medications, Allergies, and Food Habits.
