HTML Assessment Report PDF Template

I'm looking for an HTML template I can use to generate an Assessment Report PDF (using API calls delivering the dynamic data). The template should include:

- Header banner
- Title page
- Table of contents
- Executive summary
- Results tables for scores by category (dynamic data fields)
- Visualizations of assessment data (dynamic data fields)
- Open feedback responses & next steps (dynamic data fields)
- Name, counts of responses, etc.

Ideal skills and experience:
