Convert Word Form to Tally.so

I need my 80-question survey, currently sitting in a single Word document, rebuilt inside Tally.so. Every item is a rating-scale question, so there are no images, videos, or other special media to worry about—just clean, consistent scales that need to look and behave exactly as they do on paper.

You will receive the original DOCX and any notes I have on preferred scale labels. Please replicate the structure in Tally, double-check that the response options line up correctly, and deliver a shareable Tally.so link (or transfer ownership to my workspace once complete). If you spot opportunities to streamline the flow with Tally’s built-in logic or page breaks, feel free to suggest them; otherwise a straight, faithful conversion is all that’s required. I then need the data to be pulled into a visual graphic for easy interpretation.

When you’re done I’ll run through the form, confirm every scale works, and sign off.
