Simple Disease Detection AI Model

I need an AI model to define possible diseases based on symptom descriptions (primarily). Numerical data analysis such as test results (blood pressure, blood sugar, general blood test) is also preferable. Model should be based on Mixture-of-Experts (MoE) architecture where based on a type of data preprocessor (router) will direct analysis to a specific "expert", for example expert one - numeric, expert two - categorical, expert three - test data. And then prediction will be formed. This model will be used in app with adaptive questionnaire mechanism - where user will receive clarifying questions to narrow diagnosis predictions.
