I want someone to conver pdf document into proper excel fromat -- 2

I want to someone to convert pdf to a proper excel document without using pdf converters as I used it and it didn't work out for me. I have attached the format I want the document in and a Pdf file for using it. Please note I have made sheets with different qualifications. I want subjects of each qualification in one sheet. You can find the qualifications in the footer of the pdf documents.
