interpret clinical referrals and fill out relevant investigation forms

I am looking for an experienced professional to interpret a high volume of clinical referrals and fill out relevant investigation forms accordingly. Specifically, I need this person to interpret referrals that are related to medical services. I already have the necessary format for the investigation forms available, so they can simply utilize that. All in all, I need this project completed for more than 20 referrals and forms.
