intake forms and fill pdf’s

Im trying to find a software or playform that will allow me to use my intake form that i can use in my ipad or phone or computer and take that data and be able to map it to the fields in other pdf documents that require some of the information from the intake form. these will reduce the repetition of the same data such as name , age , etc.

it has to be customizable , i seen many platforms that offer similar to what i want to do , what is best recommended?
