employee evaluation score excel with document storing -- 2

I have an evaluation form in excel format for my employees. The higher ranked employee will write down his comments and scores in each of 34 total questions. I want to be able to upload the evaluation of each employee in a cell and open upon click all of their previous evaluations. And also extract the scores from each evaluation in order to filter my employees with their scores.
