PDF form and submit button with file name renamed as some text input fields

My project is to develop a PDF form and submit button for collecting data. I have a specific template in mind that I can provide as a sample. The data I want to collect using the PDF form will include both quantitative and qualitative data for waste management risk assessments. Additionally, I would like the submit button to rename any submitted files with some input from text fields. I'm looking for assistance with this project, and anyone with the necessary experience and skill is welcome to apply.
