Google sheets - Form, Sheets & Docs - Link them and generate a PDF

I need someone to help me link my Google sheets, form, and docs, and generate a PDF. The ideal candidate should have experience in Google Suite and be able to work with the following requirements:

- Google Form should include multiple-choice fields
- Google Sheets should have simple calculations automation and Vlookup
- Google doc Template
- The generated PDF should be in PDF format.
