FormSight BI System” A modular Power BI solution for extracting, analysing, and visualising Microsoft Forms data across multiple programs
Budget: £20 – £250 GBP
Overview
The APD is a data visualization solution that connects 24 Microsoft Forms to 4 separate Power BI dashboards, each designed to serve a specific program area (e.g., Staff Management, Building Operations, Client Services, and Group Activities). Each dashboard must include multiple pages for deep drill-down analysis and be packaged in a way that allows it to be **uploaded and reused across different SharePoint/OneDrive environments** with minimal reconfiguration.
---
Project Objectives
* Extract live or regularly updated data from 24 Microsoft Forms.
* Structure data into 4 Power BI dashboards, each summarising a group of forms.
* Create multiple visual pages per dashboard, showing:
* KPIs
* Time-based trends
* Status summaries
* Response heat maps or tables
* Ensure visual clarity, clean UI/UX, and proper use of slicers and filters.
* Package the solution so that it can be replicated on new SharePoint/OneDrive setups with minimal effort — i.e., a “plug and play” system.
---
Data Structure
| Dashboard Name |Form Count | Purpose |
| ----------------------------------------------------------------- | ----------------- | --------------------------------------------------------------------- |
| **1. Staff & HR Dashboard** | 4 | Sick leave, Supervision, Annual Leave, etc. |
| **2. Client Services Dashboard** | 6 | Individual client forms, referrals, goals |
| **3. Group Program Dashboard | 10 | Group sessions, outcomes, attendance |
| **4. Building & Maintenance Dashboard | 9 | Room checks, health & safety, inspections |
Each form submits responses to an Excel file stored in SharePoint/OneDrive, which Power BI connects to.
---
Dashboard Requirements (per dashboard)
Each dashboard should include **3–5 pages**, with consistent navigation and visuals, including:
**1. Overview Page**
* Total submissions
* Completion rate
* Issues flagged
* Last submission date
**2. Breakdown Page**
* Filter by form, category, or department
* Trends over time
* User/room/participant-level breakdowns
**3. Insights & Action Page**
* Most common issues
* Outstanding tasks
* Summary of flagged responses
**4. Deep Dive (optional)**
* Raw response table
* Export-ready format
* Drill-through by form
**Design Guidelines**
* Clean, minimalist layout (white background, modern font)
* Colour-coded by category (HR = Green, Client = Blue, Group = Orange, Building = Grey)
* Consistent page headers and slicers across all dashboards
* Easy-to-read KPIs and tooltips on graphs
---
**Reusability / Plug & Play Requirements**
The system must:
* Be exportable as .pbit (Power BI Template files) that allow changing the data source path (OneDrive or SharePoint folder)
* Use relative paths or parameterized source settings to swap data sources
* Come with a simple user guide for:
* Connecting to a new SharePoint/OneDrive Excel folder
* Refreshing data
* Publishing to Power BI Service or Teams
* Changing data heading
---
**What You Will Provide to the Freelancer**
* Links to all 24 Microsoft Forms
* Example Excel output for each form (or current connected Excel file)
* Labels, categories, or fields you want emphasized
* Sketches or sample layouts if available
* Branding/logo for dashboards
---
**Expected Deliverables**
* 4 fully designed Power BI dashboards (.pbix and .pbit)
* Clean data model per dashboard (grouping relevant form tables)
* Configurable SharePoint/OneDrive path settings
* Documentation PDF or Word guide:
* How to connect each dashboard to Excel files
* How to publish dashboards
* How to update or duplicate the system
