Form edit

I would like a few edits done on this form. For starters take Boston’s children’s hospital off and the logo that’s on the top left and replace it with “ Children’s MN” but exactly like it shows on the website with the star for the letter I please. Then replace the web address under the logo with this one https://www.childrensmn.org/ then on the bottom replace boston children’s hospital with Children’s MN with the logo with the star in place of the letter “I” year will be 2021 publication date will be 6/24/2021leave page 1 of 1 and remove #162332
