Data Visualization Expert

i need someone to visualize Data for me and put it in a Format so that i can produce executive summary out of it. I give you an example: I am producing leads via social media campaigns for financial advisors. These leads are people who give me sensitive financial information about them. I want to put these data points (at the moment we’re collecting about 50-100 data points on each customer) to be visualized in a way that you can easy access the information about the prospect at the end.
So all of the information we have in a spreadsheed need so go into a single pdf file that we can send to our clients at the end.
I have given you an example of how the finished product should look like in the attached file.
For example:
A Person fills out their name pretty late in our process, say for example its in column 50 but it needs to be put in our file at the beginning.
If the project suits you and you will be working with us, i am happy to provide an english file or explanation of the names and functions of the various fields.
