Data Gathering in Excel Sheet -- 2

Read attached sample before you proceed , My project goal is to gather data and organize it in an Excel sheet. To complete the project, I'll be entering the data manually. I have specific instructions in terms of how the data should be organized, so this is important to consider. I'm looking for someone to take on this task and finish the job in 1-3 days.
