U.S. County Data Map Visualization

I have data with % change in unemployment at the U.S. country level. I need someone to help me put this in a heatmap and visualize it at the county level. I have the names of the states and the counties, but I don't have any zip codes or anything like that.

The data will look like this:
State County % Change
Alabama Autauga County -26.9%

I've provided a sample output I'm looking for.
