Scanned Image Form Entry -- 2

I have a collection of scanned forms that need to be keyed into a clean, structured file. Each form follows the same layout, so once the first few are understood the rest should move quickly. Your task is to transcribe every field exactly as it appears, double-checking for typos and ensuring dates, numbers, and names are entered consistently.

I will supply the scans as high-resolution PDFs or JPGs—whichever you prefer. When the entry phase is finished, I’ll need the data returned in a single file. Excel, CSV, or Word all work for me; let me know which format you’re most comfortable with and I’ll confirm before we start.

Accuracy is more important than speed. Please flag anything unreadable rather than guessing, and keep the information confidential. If you have experience with OCR tools like ABBYY FineReader or Adobe Acrobat, feel free to use them, but a manual review is still required so every field is perfect.

Deliverable:
• Completed data file in the agreed-upon format, ready for immediate use.
