Mixed Data Entry Cleanup -- 2

I have a collection of records in our internal database that combines text fields with associated numerical values. I need every row transferred and neatly organized into a single Excel spreadsheet, matching the field order and naming conventions I will provide at the start of the project.

Accuracy matters more than speed: totals must reconcile, text spelling must stay exactly as stored, and any blank or uncertain cells should be flagged rather than guessed. Please keep source formatting such as leading zeros or currency symbols intact where they appear.

Deliverable: one .xlsx file ready for immediate import into our reporting tool, with a quick summary sheet that shows record counts and any flagged items for my review.
