Google Forms for Training Programs

I need 1 Google Form built for our online training programs. Each form should open with a brief Introduction/Instructions section, flow into the main questions, and close with a Feedback/Survey segment. I will provide the wording; you structure it cleanly so everything feels intuitive on desktop and mobile.

Question mix
– Multiple choice for quick selections
– Short answer where learners can elaborate
– Rating scale items to capture satisfaction and confidence levels

Please enable these built-in features:
• Automatic email notification to me for every submission
• A friendly submission confirmation message to participants

Once you finish, transfer ownership to my Google Drive and share the live links so I can test them. I will mark the job complete when I can submit a dummy response, see the confirmation message, and receive the email alert for all three forms.
