Dental Lab Excel Form must be convertable to pdf and or HTML in the 2nd fase

I need an Excel workbook that lets our dentists fill out a simple, structured form so we can pass clear “patient teeth requirements” on to the dental lab.

Scope
• One clean data-entry sheet styled like a form (labels, input cells, optional drop-downs) for patient and case details.
• A separate table where every submitted form line automatically lands, ready for filtering or export.
• Basic protections and formatting that keep the layout intact but still allow staff to type quickly.

I’m aiming for a straightforward setup—no complex macros, dashboards, or analytics—just a reliable form and an organised data log we can start using right away.
