Build Heatlh Assesment Jotform for Medical Clinic

I need a Jotform specialist to improve existing Jotform to allow calculations, conditional logic and better email notifications. The build must:

• Employ conditional logic, calculated fields, and hidden sections so patients see only the questions relevant to them.
• Present a clean, professional, clinical interface that feels familiar to both patients and staff.
• Generate branded PDF summaries and email notifications automatically.
• Save completed submissions to our chosen destination—Google Drive for now, with the structure ready for future EHR integration.

Familiarity with medical terminology is essential; the questions involve risk factors and screening indicators that must be worded and scored. I will supply the question set and scoring rules. Your role is to turn that content into a fully working, error-free Jotform, test every pathway, and hand over a form link plus an export clone.
