excel spreadsheet

I'm looking for someone to help me create a new Excel spreadsheet for my sales team. The spreadsheet will contain financial data, and I have a specific format in mind for how the spreadsheet should look and function (see attached). I'll need the spreadsheet to include specific calculations or formulas, check boxes and auto dates so knowledge of advanced Excel functions is required.
