Transcribe Digital Patient Records

I need help turning a backlog of patient records—currently saved as plain digital text files—into clean, accurately transcribed documents that can drop straight into our EMR system. All files are already typed, but they vary in structure, terminology use, and formatting. Your job is to review each record line-by-line, verify medical terminology, standardize headings, correct any typos, and flag inconsistencies while keeping HIPAA-level confidentiality front-of-mind.

You’ll receive batches of files via secure cloud share, and I’ll be available for quick clarifications; I have 10 years of clinical experience myself and can answer content-related questions fast. I’m looking for a detail-oriented medical transcriptionist who communicates clearly and can return polished documents in the agreed template (Word or Google Docs) with change-tracking enabled.

Deliverables:
• Fully transcribed and formatted patient records, error-free and ready for EMR import
• A brief log noting any unclear terms or missing data you encountered
