Submit Google Form Verification Proof

I need a quick, accurate helper to complete one Google Form for me and bring back proof it was done. I’ll place the form link inside the shared document you receive as soon as we start.

Here is exactly what you will do:

1. Open the link, answer every required question, and be sure the username you type matches your Freelancer username character-for-character.
2. Hit Submit.
3. Read the custom confirmation text that appears; it shows a unique verification code used solely to verify form submission.
4. Capture a full-screen screenshot of that confirmation page.

Deliverables I expect back from you:
• The exact username you entered in the form
• The unique verification code that appeared after submission
• The screenshot of the confirmation page (PNG or JPG)

Once I receive these three items and confirm the code, the task is complete.
