Replicate existing form in WordPress using the Quform WordPress plugin

I have an existing form that I need recreated in WordPress using the Quform plugin. I already have the WordPress site setup with the Quform plugin installed. I just need someone to duplicate the existing form that’s on another platform inside of WordPress using the Quform plugin and verify that everything is working properly.
I’ll provide you with the link to the old form along with WordPress access to the new form for you to setup the form. This is a very simple job for someone that’s familiar with WordPress. It’s a basic data entry job. You will also need to verify that the form is functioning as expected upon completion.
