PDF Form Recreation with Signature

I have an existing form that I need rebuilt from scratch as a fill-able, print-ready PDF. The finished file must mirror the original layout perfectly while adding a dedicated signature field that works on desktop and mobile PDF readers. I chose a PDF specifically—no Word document or web-form substitutes—so the integrity of the design and automatic field behaviour has to be preserved.
You’ll receive the original file (image or scan) as soon as we start, and I’m happy to clarify fonts, colours, or any other styling details to ensure the recreated version looks identical.
Deliverable
• One fully interactive PDF that can be saved after filling, with a functional signature field placed exactly where indicated.
Acceptance
• Visual match to the source form.
• All text boxes, checkboxes, and the new signature field behave correctly when tested in Adobe Acrobat Reader.
