Budget: $30 – $250 USD
I need to create a weekly KPI report and store all the data onto another worksheet so it is easily accessible. I have a submit button so that all the data gets transferred into the Data Storage worksheet and the Data Entry worksheet gets erased. The following parameters are needed for the KPI Report
1. Week Number(1-52)
2. Shift (1st, 2nd,3rd,4th)
3. PT Name
4. Day of Report
5. Total Washed (will be manually input)
6 Total Hours Worked (manually input)
7. Line Rate (hours worked/total washed)
8. Average FPY% for CCU (manually input)
9. Average FPY% for HSC (manually input)
10. Average FPY% for CCP (manually input)
11. Average HSC Calibration time (manually input)
12. Cleanliness (manually input)
13. Top 3 down equipment into 3 sections
14. Reasons for being off target for all FPY platforms
15. Half doughnut graphs for all FPY platforms
16. Overall Score Percentage will be weighted as follows
a) Total Washed - 40%
b) FPY% - 30%
c) HSC Calibration Time - 10%
d) Cleanliness - 20%
