Mail Merge

Hello,
I have a mail merge project for 73 addresses. The paper the addresses will print it on is a 6.14" x 9.21" portrait.
It will have the return address and the mailing permit at the top. These will remain constant. The shipping address is dynamic.
The square ( mailing permit) must be bigger than a one-half inch high by one-half inch wide. The square ( mailing permit) must place it no more than 1.5 inches below, or to the left of, the upper right-hand corner. You must create the square and fill it in using the example attached.
The addresses will be exported using PDF and Word Docs.

This job must begin immediately and be done within an hour. I will send the address (csv file) over once the bid is accepted.
