List building for marketing campaign

I'm looking for a freelancer to scrape data from Washington state electrical permit applications
to be used for marketing.
Requirements:
- Details important - full name and address must be collected with permit# and date
- List to be built so that date merge can be incorporated for printing labels.
Ideal Skills:
- familiar wih Microsoft office
eXAMPLES OF iNPUT SCREEN AND DATA SCREEN ATTACHED.
All required licenses and dates will be provided
