JotForm Expert Needed for Form and PDF Creation
Budget: $30 – $250 CAD
I'm looking for an experienced professional to create several forms on JotForm for data input on an iPhone. The input fields will be sourced from an existing PDF that we're converting into a JotForm.
Key Requirements:
- Create JotForms incorporating Text fields, Dropdown menus, and Checkboxes from the original PDF.
- Implement conditional logic to show/hide fields based on responses.
- Add our Company logo to the forms for branding purposes.
- Use JotForm's PDF Creator to format the outputted PDF to closely match the layout of the original document.
