Input a Survey in SurveyCTO

I am looking for someone to help input a complex, multi-level, detailed survey into SurveyCTO. The questionnaire is available in a Word document, and all questions and codes need to be translated so that it is ready for input into SurveyCTO. Looking for an immediate requirement, to complete this work within the next one week.
