ICD-10 Inpatient Coding Expertise
Budget: ₹100 – ₹400 INR
I need a detail-oriented coder to translate a batch of inpatient medical records into precise ICD-10 codes. The charts are already fully documented; what I require is accurate code assignment, sequencing, and any necessary laterality or encounter qualifiers so that billing, analytics, and compliance reporting are airtight.
You will receive scanned discharge summaries, operative reports, and physician notes in PDF. I expect you to return:
• A spreadsheet (Excel or Google Sheets) listing each record, the primary ICD-10 code, secondary codes, and brief clinical justification
• A short discrepancy note whenever documentation is ambiguous or insufficient
All codes must follow the latest ICD-10 CM guidelines for inpatient settings, including Present on Admission indicators. Turnaround is one week from receipt of the files, and I am available for quick clarifications throughout the project.
