Health Questionnaire Consolidation

Objective: Consolidate the questions and answers from national health surveys and health studies to thoroughly assess what will be the right questions to ask for our health survey.

Actions: Extract the question and answers from the national health surveys of the UK, and Singapore and from Precision Nutrition deep health questionnaire. The links to each of these can be found in the template provided.

Copy and paste the questions and all possible answers, plus notes, from each of the above into the Google sheet provided.

Follow the example format shown on tab 2, put one health survey on each tab, and every answer has its own column in the sheet.
