Google Form Survey Creation

I already have a set of survey questions laid out in Canva and all the content is pure text. Your task is to transpose every question, description line and optional help text into a clean, well-structured Google Form that looks and behaves exactly as intended.

Key points
• All questions are text-based; no numerical calculations or file uploads required.
• The finished Form must follow the same order, wording and section breaks shown in my Canva draft.
• I am not in a rush, so you can take the time you need to format each field properly, add page logic where relevant, and double-check for typos before handing it over.

Deliverable
• A shareable Google Form link with editing rights transferred to me, fully populated with the provided text and ready to distribute as a survey.
