I need a regular PDF converted into a fillable PDF. The fillable PDF should include:
- Text fields
- Checkboxes
Text fields should be formatted as plain text. No conditional logic is required.
Ideal skills and experience:

- Ability to deliver high-quality, user-friendly fillable PDFs
The document is on one 8.5" by 11" sheet of paper.
I hired it out yesterday, and it came back reduced to the point that my bank would not accept it. The person who did the work said she did not reduce it, but it came back clearly reduced in size.
