Customer Data Entry Form

I need a straightforward data-entry form devoted to collecting customer information. For now the only mandatory fields are the customer’s name and their contact details (phone and/or email). The form should present a clean, uncluttered interface, guide the user with basic validation, and save each entry so I can review and export the records later.

If you already have a preferred stack—whether that’s a lightweight HTML/CSS/JavaScript page, an Excel/VBA sheet, or a small desktop utility—feel free to suggest it; I’m flexible as long as the finished form is easy to deploy and use. Clear field labels, a logical tab order, and error prompts that prevent incomplete submissions will be key acceptance criteria.
