Copy data on orchestral musicians from PDF to Spreadsheet

The attached PDF lists every musician who played for the St. Louis Symphony between 1907-1908 and 1978-1979. I would like the data copied into a spreadsheet. To illustrate the desired format, I have entered the first 11 rows into the attached spreadsheet.

The PDF is 34 pages long and I estimate it lists 1500 musicians. I would estimate the job will take 15 person-hours to complete.
