Convert PDF to Fillable Mobile Form

I'm looking for a professional who can convert a PDF form into a fillable form compatible with iPads and Android devices. The fillable form should include:
- Text fields: for general input
- Check boxes: for selection options
Special formatting is required for some of the text fields, specifically:
- Date format: for any field that requires a date
Once the form is filled out, it will primarily be used to be emailed. Therefore, the form should be designed to facilitate easy emailing of the completed form.
