Attendee Form for Fashion Show
Budget: $10 – $30 USD
I need a clean, professional-looking form that captures only the personal details of people attending my upcoming fashion show. Guests will be sending their completed form back to me by email, so the file you create has to work smoothly in that context—no web portals or mobile apps this time.
Here’s what matters most to me:
• The fields must cover standard personal information (full name, email, phone, mailing address) and include an email field that validates format in real time.
• As soon as a guest hits “Submit” or “Send,” they should receive an automatic confirmation so we know the address is active.
• The design needs to match a high-end fashion aesthetic: clean typography, subtle color palette, space for our logo, and a layout that looks just as good on a laptop as it does on a phone screen.
• Final deliverable can be an interactive PDF or an HTML email-friendly form—whichever you recommend for the smoothest email submission and built-in email verification.
