I am looking for a freelancer to collect **1000 VERIFIED (active) email addresses of doctors in the USA**. Each record must include the following information (mandatory):
* Full Name
* Degree / Title (e.g., MD, DO)
* Specialty
* Hospital / Clinic Name
* Location: Address, City, State, ZIP
* Email (verified and active)
* Phone (if available) — optional
* Source URL (hospital profile / state medical board / official registry, etc.)
* Verification status (e.g., verified via SMTP / third-party tool)
* Notes (if any)
**Requirements:**
1. All data must come only from **publicly available sources** (hospital websites, NPI registry, state medical boards, LinkedIn, etc.). No purchased lists or illegal databases.
2. Emails must be **verified** using a reliable verification tool or SMTP check. Provide proof of verification (report or results).
3. Each record must include the **source URL** for manual checking.
4. Submit a **sample of 50 records first** for approval before completing the rest. No payment will be made without this verified sample.
5. Do not collect sensitive personal information (e.g., private addresses, medical history).
6. Final file must be clean, properly structured, and duplicate-free.
**Deliverables:**
