SurveyMonkey Construction Appraisal System

I’m ready to move our crew reviews online and have chosen SurveyMonkey as the platform, so I need an end-to-end appraisal flow built inside my account. The form must capture three key areas—Work quality, Team collaboration, and Punctuality—and present them cleanly to three different rater groups: supervisors, peers, and each employee for self-assessment.

Here’s what I expect:

• A branded SurveyMonkey questionnaire with skip logic that shows only the sections relevant to each rater type.
• Consistent rating scales, a few open-ended comment boxes, and mandatory fields to prevent half-finished submissions.
• Automated email invitations and reminders, scheduled so all three perspectives are collected within the same review window.
• A results dashboard and export templates (PDF and Excel) that let me filter by employee, team, or date so I can discuss findings in one-on-one meetings.
• A short admin guide—screenshots plus step-by-step notes—so I can duplicate the survey for future cycles without outside help.

Once the draft form is live, I’ll run a test with two sample employees. I’ll sign off when each rater group can complete the survey smoothly and I can pull a consolidated report in under five clicks.
