We are conducting an academic research study in the United States on Type 2 Diabetes and are looking for individuals diagnosed with Type 2 Diabetes to participate by completing a structured questionnaire. The purpose of this study is to collect real data on lifestyle habits, disease awareness, treatment practices, and self-management behaviors among diabetic patients.
Eligible participants must be 18 years or older, diagnosed with Type 2 Diabetes, and currently residing in the United States. Participants will be required to fully complete a questionnaire designed by the researcher, which includes questions about diet, physical activity, medication use, blood sugar monitoring, family history, awareness of complications, and access to healthcare services. The questionnaire is expected to take approximately 15–25 minutes to complete.
Data will be collected from diabetic individuals in major US cities such as New York City, Los Angeles, Houston, Chicago, and Miami to ensure diversity and reliability of results. Participants may be asked to provide confirmation of diagnosis, such as a doctor’s prescription, medical report, or medication record, where available. However, self-reported diabetes status will also be accepted for participation.
All responses will remain strictly confidential and will be used only for academic research purposes. Participation is voluntary, and participants may withdraw at any time without any consequences. There are no medical procedures or risks involved in this study.
The questionnaire will be administered directly by the researcher and completed responses will be used for statistical analysis to understand patterns and factors affecting diabetes management in the United States.The deadline for completing participation in this study is 5 days from the date of recruitment.
