Online Customer Satisfaction Survey

I need a concise, well-structured online survey that will let me gauge how satisfied my existing customers in the UK truly are. The end product should make it easy for me to send a single link, collect responses in real time and view clear, actionable insights without extra hassle.
Scope of work
• Draft a set of thoughtful questions that cover overall satisfaction, recent support experience, perceived value, and likelihood to recommend.
• Build the questionnaire in a mainstream survey platform (Google Forms, Typeform, SurveyMonkey or similar) so I can access and export the data myself.
• Configure basic logic and mandatory fields to keep completion time under five minutes.
• Provide a short guide that shows me how to distribute the link and monitor responses.
Acceptance criteria
1. Survey runs end-to-end in the chosen platform, tested on desktop and mobile.
2. Maximum of 15 questions; average completion time ≤5 minutes.
3. I receive the editable form, a shareable link, and clear instructions for exporting results.
