Hospital Employment History Verification

I’m putting a short-term process in place to confirm that hospital candidate truly held the position and dates stated on their certificate. All I need is straightforward employment history verification—just official start and end dates along with the exact job titles recorded by the employer. No background or reference checks, and no deep dive into duties or reasons for leaving.
The process is in-person verification—so long as the information comes directly from an authorised representative of the former employer and complies with local POPIA privacy requirements.

Expected deliverable:
8-9 geo tagged pictures
A complete filled verification form ( that lists the verified data, the date of your contact, and the name, title, and contact details of the person who supplied the information, along with any pertinent notes.)
BRC/letterhead (if available)
A site visit Questionnaire
