I am compiling an authoritative reference that focuses exclusively on medication side effects, and I need help extracting and structuring detailed clinical information for a wide range of prescription and over-the-counter drugs.

Scope
• Concentrate solely on side-effect data—dosage instructions, pricing, and general pharmacy details are outside this project.
• Depth matters: I’m after the kind of information you would find in clinical trial reports and post-marketing surveillance—incidence rates, severity grades, contraindicated populations, black-box warnings, and any noted drug–drug interactions that directly relate to adverse reactions.

Preferred approach
You may pull from peer-reviewed journals, FDA labeling, EMA summaries, and reputable pharmacovigilance databases such as FAERS or MHRA Yellow Card. A short citation for every data point is required so the content can be verified quickly.

Deliverable
A structured spreadsheet (CSV or Excel) that, for each medication, includes:
1. Generic & brand names
2. Therapeutic class
3. Documented side effects grouped by body system
4. Frequency (common, less common, rare) with numerical incidence where available
5. Severity (mild, moderate, severe)
6. Special population notes (pregnancy, pediatrics, geriatrics)
7. Source citation (URL or DOI, plus publication year)

Acceptance criteria
• Minimum 200 distinct medications covered.
• At least two reputable sources cited per drug.
• Data passes a random spot-check for accuracy and proper citation.
