Text Data Entry into Excel

I have a collection of text records that must be keyed accurately into an Excel spreadsheet. The workbook already contains the exact headers and column order I need maintained, so your job is simply to transcribe each entry under the right heading without altering the template.

Here is what I will supply:

Here is what I expect back:
